Aceclofenac

Indications

Aceclofenac is prescribed for the relief of pain and inflammation associated with osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, low back pain (lumbago), toothache, and soft-tissue injuries or trauma.

Pharmacology

Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) with both anti-inflammatory and analgesic properties. It works primarily by inhibiting the enzyme cyclooxygenase (COX), which plays a key role in the synthesis of prostaglandins—substances responsible for pain, swelling, and inflammation. After oral administration, Aceclofenac is rapidly and completely absorbed and remains largely unchanged in the bloodstream.

Dosage & Administration

  • Extended-release tablet: The usual adult dose is 200 mg once daily, or as directed by the physician.
  • Film-coated tablet: The usual adult dose is 100 mg twice daily.

Drug Interactions

No significant drug interactions have been commonly reported; however, careful monitoring is advised when Aceclofenac is administered with the following:
  • Lithium or Digoxin: May increase serum concentrations of these drugs.
  • Diuretics: May reduce the effectiveness of diuretic therapy. Anticoagulants: May enhance the anticoagulant effect, increasing the risk of bleeding.
  • Methotrexate: May elevate plasma levels of methotrexate, increasing toxicity risk.

Contraindications

Aceclofenac is contraindicated in patients with:
  • Known hypersensitivity to Aceclofenac or other NSAIDs.
  • A history of aspirin- or NSAID-induced asthma, urticaria, or allergic-type reactions.

Side Effects

Common side effects may include nausea, indigestion, abdominal pain, dizziness, rash, or headache. Gastrointestinal disturbances such as ulceration or bleeding may occur in sensitive individuals or with prolonged use.

Pregnancy & Lactation

Aceclofenac should be avoided during pregnancy and breastfeeding, unless the potential benefit to the mother clearly outweighs the possible risks to the fetus or infant. Use only on medical advice.

Precautions & Warnings

Caution is advised in patients with: Active or suspected peptic ulcer or gastrointestinal bleeding. Moderate to severe hepatic, cardiac, or renal impairment. A history of asthma, dizziness, or urticaria. Patients should be advised to take the medication after meals and report any unusual symptoms such as black stools, abdominal pain, or skin rash.

Use in Special Populations

The safety and efficacy of Aceclofenac in children have not been established due to a lack of sufficient clinical data.
